Oct 7 09 Correct Flu declination form now on MDPH website and attached
At the insistence of MNA, the MA Department of Public Health has removed from its website a link to a vaccination declination form that the MA Hospital Association has been pushing, which MNA takes strong issue with and which DPH itself had told us they did not support (although they had the link on their website, contributing to confusion by hospitals).
Again, at MNA’s request, the only link on the MDPH’s flu website (below) now is to the form that MDPH and MNA both support.
