Revised Patient SRE Letter Template – for reading level

We are writing to say how sorry we are that you experienced _________ during your stay in our hospital.  We take these events very seriously. As a result of this we will:

  • Send an initial report of this event to the Massachusetts Department of Public Health (DPH), as required by law.  What you experienced is  considered a “Serious Reportable Event”.
  • Send a notice to your health insurance company.
  • Conduct an internal investigation to see why this happened, if it could have been prevented, and what if anything can be changed to prevent something like this from happening again. We may take up to 30 days to complete this investigation.
  • Send a final report of our findings to the DPH, to you, and to your health insurance company.


The Department of Public Health’s goal for reporting is to improve patient care.  If you would like a copy of the initial report that will be sent to the DPH, please call us at __________________________.
