The RN Supervisor is responsible for the management and supervision of the interdisciplinary care management team and insuring that EOEA's Interdisciplinary Case Management standards are met. With the Senior Case manager, Case Managers and other support staff, the RN Supervisor will function as a Team RN and conduct home visits to assess clients' functional, health and income status to determine eligibility and appropriateness for community long term care services and programs and coordinate services to clients.
1. Provides supervision and oversight of care management team
2. In conjunction with team members determine client appropriateness for various programs and services and participates in determining eligibility for Home Care, Medicaid funded programs and other related services and the frequency, scope and duration of services.
3. Promotes coordination and cooperation with community agencies and institutions that provide medical and social services to clients and colaborate with those agencies in the formation of appropriate, cost effective, quality service plans and cost effective service substitutions.
4. Participates in Grand Rounds and in the interdisciplinary review and assessment of cases and assists clients in the referral process.
5. Work with appropriate CBES staff and community resources to provide consultation to care managers individually and as a team regarding skills in client needs assessment, service planning, use of resources, time management, problem solving techniques and issues related to gerontology.
6. Participates in the hiring process and orientation of new care managers.
7. Assist care managers with professional growth and development and career related goals.
8. Provides consultation to caregivers, providers and other community agencies.
9. As a member of the management team, assist care managers in preparing for client appeal hearings.
10. Assists in problem solving as it relates to care management and consumer related issues.
11. Act as Step 1 Grievance Officer as per SEIU Local 509 Bargaining Unit agreement.
12. Conducts and participates in agency meetings, committees, task forces, trainings and workshops as needed.
13. Maintains electronic and other records and prepares reports as requested.
14. Provide office supervisory coverage in special situations, including assistance to clients in absence of care managers and back-up for absent supervisors.
Must possess a valid Massachusetts license to practice as a Registered Nurse
Two years of clinical experience with one year of community nursing preferred
Must have demonstrated leadership ability
Must be able to work effectively within a diverse team
Must be organized and detail oriented
Possess working knowledge of community resources and reimbursement systems for health and social services
Bi-lingual either Chinese, Russian, Haitian Creole or Spanish preferred but not required
